29 CFR 1926.20(b)(2): The employer did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:      a) Job site, 606 Cedar Avenue, Pittsburgh, PA: On or about October 2, 2018, during construction activities, the employer did not ensure that an effective system was in place to identify and correct safety and health hazards at the job site. Deficiencies noted included, but were not limited to, interior railings along walking/working surfaces that were too low to serve as appropriate fall protection, failing to ensure workers who were engaged in demolition activities wore protective headwear, inappropriate electrical splices, lights without covers, openings in temporary electrical service circuit breaker panels, lack of handrails on stairs and tripping hazards caused by either depressions in floor/missing floor boards and/or temporary electrical service knockout boxes being mounted on the floor in walking/working areas.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury:   a) Job site, 606 Cedar Avenue, Pittsburgh, PA: On or about October 2, 2018, during construction activities, the employer did not ensure that employees were provided with basic safety and health training relevant to their work environment. Deficiencies noted included, but were not limited to, failing to provide safety training on personal protective equipment where head protection was needed, not providing instruction on electrical safety where hazards such as inappropriate electrical splices, lights without covers and openings in temporary electrical service circuit breaker panels existed and not ensuring that employees were aware of the need for handrails on stairs.
