OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that an employee was exposed to a fall hazard: a) 155 Brookdale Dr., Springfield, MA: On or about October 3, 2018, an employee was observed working from a raised aerial lift (Genie S -45) with his Guardian brand lifeline/lanyard tied to itself around the rails of the aerial lift. Among other methods, feasible and acceptable means to correct this hazard is: 1. Ensure that employees are trained and follow the manufacturer's recommendation printed on the lift which state "attach lanyard to anchor provided in platform" 2. Ensure that employees are trained and follow the manufacturers (Guardian VLA-50) recommendation that, "if the shock absorber is built into the leg of the rope grab, positioning device or fall arrester, that portion must be attached to the user's approved body harness".
