Employees were exposed to slip, trip, and fall hazards from debris that had accumulated on the yard that employees would descend to. There is no abatement certification or documentation required for this item.

On or about October 4, 2018, employees were exposed to fall hazards from ladders that were not extended at least 3 feet above the upper landing surface. 29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired. 516 Andover Street, Lawrence, MA 01843 a) On or about October 4, 2018, a ladder used to access the spire of the roof was found to have a rail that was bent below the bottom rung to where the rail met the foot. Employees using this ladder to access the roof are exposed to fall hazards due to rail failure. b) On or about October 4, 2018, a ladder used to access the roof was found to be missing a rung from the lower section of the ladder. Employees using this ladder to access the roof are exposed to fall hazards due to the failure of the ladder from a missing rung. There is no abatement certification or documentation required for this item.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): 516 Andover Street, Lawrence, MA 01843 a) On or about October 4, 2018, employees were not provided with protective measures to protect from falls from height. Employees were working on a roof approximately 15'6" in height without the use of a personal fall arrest system, or any other means of fall protection. b) On or about October 5, 2018, employees were not provided with protective measures to protect from falls from height. Employees were working on a roof approximately 10'7" in height without the use of a personal fall arrest system, or any other means of fall protection. LC Construction and Remodeling Inc. was previously cited for a substantially similar violation of the Occupational Safety and Health standard, or it's equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1322225, citation number 1, item number 1, issued on June 25, 2018, and became a final order on August 8, 2018. There is no abatement documentation or certification required for this item.
