29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b) Location: Endicott Woods - 70 Endicott St., Norwood, MA 02062 On or about August 2, 2018 employees were exposed to fall hazards to the ground below from roofs, where adequate fall protection was not used. Instance #1 - Building 10: Two employees were exposed to a fall of approximately 27 feet to the ground below, while engaged in roofing activities. Instance #2 - Building 9: Two employees were exposed to a fall of approximately 12 feet to the ground below, while working off of the roof.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. Location: Endicott Woods - 70 Endicott St., Norwood, MA 02062 On or about August 2, 2018 employees were exposed to fall hazards to the ground below from the roofs, where the roof anchors, installed and used by employees engaged in siding, trim installation and roofing activities, were not capable of supporting at least 5,000 pounds (22.2 kN) and where the anchors used were not part of a complete personal fall arrest system that maintains a safety factor of at least two and were not designed, installed, and used under the supervision of a qualified person. Instance #1 - Building 10: Two employees were exposed to a fall of approximately 27 feet to the ground below, while engaged in roofing activities. Instance #2 - Building 9: Two employees were exposed to a fall of approximately 12 feet to the ground below, while working off of the roof.
