29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: (a) On or about 10/04/2018 at the maintenance shop, employees were exposed to a struck-by hazard, in that, the portable grinder did not have the work rest properly adjusted (1/8") when the grinding wheel (stone) is worn down, creating a wider gap between the machine and wheel.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a) On or about 10/04/2018 at the maintenance shop, employees were exposed to a struck-by hazard, in that, the portable table grinder did not have the tongue guards properly adjusted (1/4") when the grinding wheel (stone) is worn down, creating a large wider gap between the machine and wheel.
