29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about October 1, 2018, the Lavatec Laundry Folding Machine (Blanket Folder #2) was not properly guarded at the input and output work stations of the machine, thereby exposing employees to ingoing nip points and rotating parts. (b) On or about October 1, 2018, the Lavatec Laundry Folding Machine (connected to Ironer #2) was not properly guarded at the output work stations of the machine, thereby exposing employees to ingoing nip points and rotating parts.

29 CFR 1910.264(d)(2)(i)(a): Operating Rules, Mechanical, Safety Guards. No safeguard, safety appliance, or device attached to, or forming an integral part of any machinery shall be removed or made ineffective except for the purpose of making immediate repairs or adjustments. Any such safeguard, safety appliance, or device removed or made ineffective during the repair or adjustment of such machinery shall be replaced immediately upon the completion of such repairs or adjustments. (a) On or about October 1, 2018, in the clean laundry department, safety interlock was made ineffective on the Jensen Press A (the press on Tunnel A) which exposed employees to crushing hazards.
