29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On 10/05/2018, at the job site located 3714 Wright Ave., Saint Ann, Missouri, a worker using nail gun to affix shingles on the roof did not use protective eyewear.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:(a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS)(DESCRIBE HAZARD(S) WHERE NECESSARY)  On or about 10/05/2018, on a roof of a church located at 3714 Wright Ave., Saint Ann, Missouri, employees engaged in roof repair work were not protected from falling to the lower level. The workers were not wearing personal fall arrest system, but were using ropes to pull themselves to the top of the roof.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  On or about 10/05/2018, the employer did not conduct frequent and regular inspection of the work site to ensure that the workers were protected from falling. The employees were engaged in removing the shingles and replacing them with new ones for five days. The workers stated that they had not seen the owner of the company at all.
