29 CFR 1926.501(b)(10): Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 20-25 feet above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a) Weathersure Systems, Inc, 4780 Vasquez Blvd, Denver, CO 80216: On and before October 4, 2018, the employer did not ensure that each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet or more above lower levels was protected from falling by a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Employees were performing roofing work without adequate fall protection.This condition exposed employees to fall hazards.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards: a) Weathersure Systems, Inc, 4780 Vasquez Blvd, Denver, CO 80216: On and before October 4, 2018 the employer did not provide a training program for each employee who might be exposed to fall hazards. Employees were not trained on the entire fall protection system being utilized at the site. This condition exposed employees to fall hazards.
