29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:     130 Goddard Memorial Dr., Worcester, MA: On October 5, 2018 and at times prior, employees performing maintenance and servicing on machines did not utilize the documented energy control procedures.  On October 5, 2018, an employee performing maintenance on a pneumatic component on a machine removed the quick-disconnect coupler to the pneumatic energy source but did not apply the isolating device and lock.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.  130 Goddard Memorial Dr., Worcester, MA: On October 5, 2018 and at times prior, the employer's energy control procedures did not specify the steps to shut down, isolate, and secure machines to control hazardous energy.  The employer developed a two-page hazardous energy control inventory for approximately thirty machines and the inventory listed the same method (disconnect at machine) to control electrical and pneumatic energy sources for most of the machines.  Some machines had multiple electrical disconnect locations to choose from and machines either used quick disconnects or lever ball valves to isolate pneumatic energy and the procedures did not specify using isolation devices or locks.  On October 5, 2018, an employee performing maintenance on a machine only disconnected the pneumatic energy source as was listed in the procedure.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    130 Goddard Memorial Dr., Worcester, MA: As of October 5, 2018 and for eighteen months prior, the employer had not performed a periodic inspection that ensured each procedure was inspected for inaccuracies during an actual energy control task.  The employer also did not ensure that the inspector reviewed each authorized employee's responsibilities under each energy control procedure.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:    130 Goddard Memorial Dr., Worcester, MA    On October 5, 2018 and at times prior, the employer did not ensure that an employee who voluntarily used a half face elastomeric respirator when deburring metal parts was medically able to use the respirator and that the respirator was properly cleaned, stored, and maintained.
