29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    Employees were exposed to struck by hazards.  Roofers were exposed to eye injury while installing roofing materials with a pneumatic nail gun.  Eye protection was not provided by the employer.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Employees were exposed to fall hazards.  Employees involved in roofing activities were exposed to a fall hazard of 11 feet from eave of the roof to the ground below in that the employees were not using fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Employees were exposed to a fall hazard.  Roofers were using an aluminum ladder for access and egress from the roof which extended approximately 1.5 feet above the upper landing surface.  Employees were exposed to a fall distance of approximately 11 feet from eave of the roof to the ground below.
