29 CFR 1910.25(b)(3): The employer did not ensure that the stairs had uniform riser heights and tread depths between the landings: a. On or about October 10, 2018, at the Hank Wang Laser, the stair riser height was not uniform between the stair case and the landing area at the machine table. Employees used the stair case to access the machine table and were exposed to slip and trip hazards.

29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work shift to employees when they were in their work area(s): a. On or about October 9, 2018, establishment, the employer did not have a safety data sheet for Vinaflex, which contained hazardous chemicals including but not limited to Polyvinyl Chloride Resin. Employees were exposed to hazardous chemicals when laser cut the Polyvinyl Chloride containing materials.
