29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) On or about 10/05/18, at a 2-story house located at 580 Auburn Ave., Buffalo, NY: An employee was standing on the aluminum pic of a ladder jack scaffold erected on the west side of the house while replacing a facia board. The employee was exposed to a fall of approx. 16 ft. to the ground because no fall protection system was utilized. NO ABATEMENT CERTIFICATION REQUIRED

No fall protection system was utilized. The roof pitch was approx. 12 in 12. NO ABATEMENT CERTIFICATION REQUIRED
