29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:   a) On or about 10/05/18, at a 2-story house located at 580 Auburn  Ave., Buffalo,  NY:  An  employee was standing on the aluminum pic of a ladder jack scaffold erected on the west side of the house while replacing a facia board.  The employee was exposed to a fall of approx. 16 ft. to the ground because no fall protection system was utilized.     NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) On or about 10/05/18, on the west side of the roof of a 2-story house located at 580 Auburn  Ave., Buffalo, NY: Three employees engaged in roof sheeting work on a steep roof  were exposed to a fall of approx. 18 foot.  No fall protection system was utilized. The roof pitch was approx.  12 in 12.   NO ABATEMENT CERTIFICATION REQUIRED
