29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 2 Item 2 for hazards associated with lack of training on fall hazards and fall protection while employees were performing roofing activities.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about October 10, 2018, employees were exposed to a fall of approximately 20 feet while performing framing activities on a new residential home located on SL 22 Salt Creek Run, Peninsula, Ohio 44264. The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about October 10, 2018, employees were exposed to a fall of approximately 20 feet while performing framing activities on a new residential home located on SL 22 Salt Creek Run, Peninsula, Ohio 44264. The employees were using air powered nail guns and the employer did not ensure that they were wearing safety glasses.   Dutch Heritage Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.102(a)(1), which was contained in OSHA inspection number 1191183, citation number 1 item number 1 and was affirmed as a final order on 12/13/2016, with respect to a workplace located at 508 Sheffield Ct., Aurora, Ohio 44202.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  On or about October 10, 2018, employees were exposed to a fall of approximately 20 feet while performing framing activities on a new residential home located on SL 22 Salt Creek Run, Peninsula, Ohio 44264. The employees were working on all areas of the roof and the employer did not ensure that they were trained on how to recognize and minimize fall hazards.  Dutch Heritage Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.503(a)(1), which was contained in OSHA inspection number 1339724, citation number 1 item number 1 and was affirmed as a final order on 9/19/2018, with respect to a workplace located on SL 279, Hooper Drive, Medina, Ohio 44256.
