29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 1 for employees exposed to overhead hazards and lack of protective helmets, Citation 2 Item 1 for employees exposed to fall hazards and lack of fall protection, and Citation 3 Item 1 for employee exposed to eye hazards and lack of eye/face protection.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about July 8, 2019, employees were exposed to a struck by hazard while performing framing activities on a residential home located at 8475 Eden Ct, Streetsboro, Ohio 44241.  There were employees working underneath other employees where there was the possibility of being struck by falling objects and the employer did not ensure that they were wearing hard hats.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels are not protected by guardrail systems, safety net system, or personal fall arrest system, nor are employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).   The employer does not protect each employee from fall hazards while performing framing activities on a residential home with unprotected edges at heights of approximately 20 feet above a lower level by the use of guard rail systems, safety net systems, or personal fall arrest systems.   a) This most recently occurred on July 8, 2019, at 8475 Eden Court, Streetsboro, Ohio 44241.  Employees are exposed the hazard of falling approximately 20 feet to the ground, while involved in framing activities near the unprotected edges of the upper levels of the residential home.    To abate the hazard in the future, the employer must ensure that its employees are protected from the hazard of falling while performing framing activities along the unprotected edges of the residential home.   Pursuant to 29 CFR 1903.19 the employer must submit documents describing the steps it is taking to ensure compliance, including an explanation of how these steps protect its employees from fall hazards.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about July 8, 2019, employees were exposed to a struck by hazard while performing framing activities on a residential home located at 8475 Eden Court, Streetsboro, Ohio 44241.  The employees were using air powered nail guns and the employer did not ensure that they were wearing protective safety glasses.  Dutch Heritage Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1191183, citation number 1 item number 1 and was affirmed as a final order on 12/13/2016, with respect to a workplace located at 508 Sheffield Ct., Aurora, Ohio 44202.  Dutch Heritage Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1352142, citation number 2 item number 1 and was affirmed as a final order on 11/21/2018, with respect to a workplace located at SL 22 Salt Creek Run, Peninsula, Ohio 44264.
