29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about October 9, 2018, employees were exposed to a struck by hazard while installing roofing materials on a new residential home located at 1215 Barker Blvd., Kent, Ohio 44240. The employees were using air powered nail guns and the employer did not ensure that they were wearing protective safety glasses.

On or about October 9, 2018, employees were exposed to a fall of approximately 9 feet while installing roofing materials on a new residential home located at 1215 Barker Blvd., Kent, Ohio 44240. The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about October 9, 2018, employees were exposed to a fall of approximately 9 feet while installing roofing materials on a new residential home located at 1215 Barker Blvd., Kent, Ohio 44240. The employees were using an extension ladder to access the roof and the employer did not ensure that it extended at least three feet above the landing area.
