29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: 7 Railroad Ave, Atrium Roof - An employee was working on a roof surface with inadequate fall arrest systems in place. Individual was not tied off allowing him to fall from a height of approximately nine feet, five inches. This condition exposed employees to a fall hazard.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: 7 Railroad Ave, Atrium Roof - An Employee was working on a roof surface with inadequate fall arrest systems in place. Individual was tied off but the rope had a big loop on it that was approximately ¾ of the way down the roof. Configuration of the equipment would allow employees to fall from a height of approximately nine feet, five inches. This condition exposed employees to a fall hazard.
