29 CFR 1926.501(b)(4): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    a) Roof of Bo AG Building, 104 N Pacific Ave, Dillon, MT: On or about October 9, 2018 and at times prior, Engineered Structures Inc., as the controlling employer, did not ensure that its sub contractor, K & K Roofing & Excavation, Inc. protected their employees in that employees were exposed to fall hazards of approximately 18 feet to the concrete below. Employees were engaged in roofing activities such as, but not limited to; installing Styrofoam roofing, clean up, and accessing/egressing the flat roof, while walking and working in and around an unguarded roof hole.
