29 CFR 1910.28(b)(3)(iv): The employer did not ensure each employee was protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used.  a) On or about 10/10/18, On the mezzanine in the Shipping and Receiving area; employees were exposed to a 9 foot fall hazard when the west opening to the mezzanine entrance was protected with a single chain attached at the top of the guard rail.
