29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about October 11, 2018, on the roof of a residence under construction located at 1531 Binney Drive in Fort Pierce, Florida, an employee was exposed to a fall hazard of approximately 11 feet - 3 inches when performing carpentry work on the roof of a residential structure while not being protected from falling to the lower level.

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall: On or about October 11, 2018, at the north roof edge of a residence under construction located at 1531 Binney Drive in Fort Pierce, Florida, an employee was exposed to a fall hazard of approximately 11 feet - 3 inches when climbing a ladder while carrying objects including but not limited to a pneumatic gun, circular saw and/or lumber that could cause him to lose balance and fall.
