29 CFR 1910.1026(c): Employees were exposed to an airborne concentration of chromium (VI) which exceeded 5 micrograms per cubic meter of air, as an 8-hour time-weighted average: (a) Chrome Shop: On October 16, 2018, an employee conducting chrome plating operations was exposed to an airborne concentration to hexavalent chromium at an 8-hour Time Weighted Average of 8.1 micrograms per cubic meter of air for the 477 minute sampling period. The employee was exposed over the permissible exposure limit of 5.0 micrograms per cubic meter of air. (b) Chrome Shop: On October 16, 2018, an employee conducting chrome plating operations was exposed to an airborne concentration to hexavalent chromium at an 8-hour Time Weighted Average of 7.3 micrograms per cubic meter of air for the 472minute sampling period. The employee was exposed over the permissible exposure limit of 5.0 micrograms per cubic meter of air. (c) Chrome Shop: On October 16, 2018, an employee conducting chrome plating operations was exposed to an airborne concentration to hexavalent chromium at an 8-hour Time Weighted Average of 7.3 micrograms per cubic meter of air for the 464 minute sampling period. The employer has not implemented all feasible engineering controls and work practices to achieve compliance with the permissible exposure limit of 5.0 micrograms per cubic meter of air. (b) Chrome Shop: On October 16, 2018, an employee conducting chrome plating operations was exposed to an airborne concentration to hexavalent chromium at an 8-hour Time Weighted Average of 7.3 micrograms per cubic meter of air for the 472minute sampling period. The employer has not implemented all feasible engineering controls and work practices to achieve compliance with the permissible exposure limit of 5.0 micrograms per cubic meter of air. Abatement Note: Feasible engineering controls include, but are not limited to the following: 1. Improve existing local exhaust ventilation to capture hexavalent chromium before reaching employees breathing zone. Step 1: Effective respiratory protection program shall be provided and used by exposed employees as an interim protective measure until feasible engineering controls and work practices can be implemented, or whenever such controls fail to reduce employee exposure to within permissible exposure limits. Step 2: Submit to the Area Director a written, detailed plan of abatement outlining a schedule for the implementation of engineering and/or administrative measures to control employee exposure to hazardous substances as referenced in this citation. The plan shall include, at a minimum, target dates for the following actions which must be consistent with the abatement dates required by this citation: (1) Evaluation of engineering/administrative control options; (2) Selection of optimum control methods and completion of design; (3) Procurement, installation and operation of selected control measures; and (4) Testing and acceptance or modification redesign of controls. Note: All proposed control measures shall be approved for each particular use by a competent industrial hygienist or other technically qualified person. Step 3: Abatement shall have been completed by the implementation of feasible engineering and/or administrative controls upon verification of their effectiveness in achieving compliance.

29 CFR 1910.1026(d)(2)(iii): The employer used the scheduled monitoring option, and monitoring revealed employee exposures to chromium (VI) to be at or above the action level; however, the employer did not perform periodic monitoring at least every six months: (a) Chrome Plating Department: On August 2, 2017 and times prior, the employer's initial exposure determination conducted in October 2015 showed an employee's exposure was greater than the action level. Additional monitoring conducted in August 2017 also showed an employee's exposure was greater than the action level. The employer has not conducted employee exposure monitoring at least every six months. Industrial Plating & Machine Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1910.1026(d)(2)(iii)), which was contained in OSHA inspection number 1250513, citation number 1, item number 7a, and was affirmed as a final order on October 20, 2017, with respect to a workplace located at 301 Princeton Avenue, Princeton, WV 24740.
