29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact:    a) Third Floor Balcony - An employee was using a Bosch hammer drill, serial number 11264EVS, to chip through a brick wall and was not wearing eye protection, on or about 10/11/2018.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:    a) Third Floor Balcony - An employee was standing on a balcony while chipping a doorway through a brick wall and the lanyard attached to the body harness was rigged so that in the event of a fall the employee could have fallen at least 9 and a half feet, on or about 10/11/2018.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
