29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On the west side of the construction site, employees were not wearing hardhats while working below roofing employees that were installing roofing material. The employees installing cornice material below was exposed to struck-by hazards from falling objects.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On the west side of the construction site, employees engaged in cornice activities used pneumatic nail guns and were not wearing eye and face protection and exposed to struck-by hazards and eye injuries.

29 CFR 1926.304(d): When the tool was withdrawn from the work, the lower guard did not automatically and instantly return to the covering position. On the west side of the construction site, employees using skill saws were exposed to amputation hazards: a) Makita skill saw #1 lower guard wedged in the up position while cutting cornice material. b) Makita skill saw #2 lower guard wedged in the up position while cutting cornice material.
