29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift. On 10/10/2018, employees from Pate Contractors Inc. were installing cable on the side of road at 500 Highway 32 East in Salem, MO.
