29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice: (a) On or about October 11, 2018, employees operating powered industrial trucks in and around the end of aisle four in the tunnel within the refrigerated warehouse area were exposed to a loss-of-control hazard in that there was an approximately 11.5 inch square smooth metal drain located approximately thirty-two (32) inches out into the tunnel passageway.
