29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a) Front Fascia: On or about October 10, 2018, and at times prior, the employer exposed employees to struck-by hazards while using pneumatic nail guns, in that protective eyewear was not provided to employees operating these tools at 120 PSI while engaged in roof trusses installation activities.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) Top of the first floor: On or about October 10, 2018, the employer exposed two employees to fall hazards of approximately 12 feet while installing flat roof trusses above the first floor of a commercial construction site without proper fall protection.
