29 CFR 1910.28(b)(3)(i): The employer did not ensure each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following: covers, guardrail systems, travel restraint systems, or personal fall arrest systems, as required by sub-items A through D of this section:    (a)  On or about October 11, 2018, and at times prior, employees were exposed to slip, trip, and fall hazards in the holes of the walking surface on the north side of Cumflow Crocker Mixers (mixer 1 and mixer 2) in the dry mold filling area, which are elevated five feet off of the ground.  The hole next to Mixer 1 was measured at 7 3/4 inches wide and 42 inches long.  The hole next to Mixer 2 was measured at 7 3/4 inches wide and 29 inches long.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  (a) On or about October 11, 2018, and at times prior, employees were exposed to fall hazards from the loading dock when loading trucks in the center where the dock is 48 inches high, and throwing trash into the dumpster on the east side where the dock is greater than 48 inches above the ground.  Abatement Note:  Abatement certification is required for this item (see the enclosed, "Certification of Corrective Action Worksheet").

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) On or about October 11, 2018, and at times prior, the projecting shaft end of the mixer on the elevated platform in the wet mold area, was unguarded.  Employees stand next to the rotating shaft when pouring contents into the mixer and are exposed to entanglement hazards.

29 CFR 1910.334(a)(2)(ii):  There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service:    (a)  On or about October 11, 2011, and at times prior, employees were exposed to shock, electrocution and burn hazards while charging the Big Joe reach truck, which had a charging cord with insulation that was sliced open and taped over.
