29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:      Instances including, but not limited to, the following:    a) Job site, 1740 Brighton Place, Pittsburgh, PA: On or about October 10, 2018, during construction activities, the employer did not ensure that appropriate fall protection systems/devices were in use where fall hazards of up to approximately 7 feet, 5 inches existed at first floor wall openings along the front of the property .    b) Job site, 1740 Brighton Place, Pittsburgh, PA: On or about October 10 2018, during construction activities, the employer did not ensure that appropriate fall protection systems/devices were in use where fall hazards of up to approximately 18 feet existed at second floor wall openings along the front of the property.    c) Job site, 1740 Brighton Place, Pittsburgh, PA: On or about October 10, 2018, during construction activities, the employer did not ensure that appropriate fall protection systems/devices were in use where fall hazards of up to approximately 12 feet, 4 inches existed at a second floor wall openings near the rear of the building.
