29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about October 12, 2018, during the installation of roofing material at a job site located at 218 Lincoln way east, Massillon, Ohio 44646; the employer did not ensure employees working on a roof of a commercial location (public library) were protected from a fall of approximately 21.2 feet by utilizing a fall protection system.
