29 CFR 1926.403(b)(2):  Listed, labeled, and/or certified equipment was not installed and/or used in accordance with instructions included in the listing, labeling, and/or certification:  a) 61 York St., West Springfield, MA: On or about October 10, 2018, an employee was observed using a circular saw which was powered through 2 orange extension cords which were daisy chained together.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or  a lanyard was not attached to the boom or basket when working from an aerial lift:  a) 61 York St., West Springfield, MA: On or about October 10, 2018, an employee was observed working from an aerial lift  (Lull 844C) basket without being attached to the boom or basket   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).  Bukowski Construction LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926. 453 (b) 2 (v), which was contained in OSHA inspection number1297251, citation number 1 item number 1 and was affirmed as a final order on 6/8/18, with respect to a workplace located at Pond View Estates, Ludlow, MA.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  a) 61 York St., West Springfield, MA: On or about October 10, 2018, an employee was observed working on the roof of this residence, more than 20 feet above the ground below without any fall protection.    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).  Bukowski Construction LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926. 501 (b) 13, which was contained in OSHA inspection number1297251, citation number 1 item number 2 and was affirmed as a final order on 6/8/18, with respect to a workplace located at Pond View Estates, Ludlow, MA.
