29 CFR 1926.25(a): Employees were exposed to slips, trips and falls from the debris left in work area. All debris shall be kept clear from work areas and passageways in and around buildings and other structures:   a) Scaffold in location of Qdoba Mexican Restaurant, 1001 East Broadway Street, Missoula, MT: On or about October 12, 2018 and at times prior, Sirius Construction, Inc. as the controlling employer, did not ensure that is sub contractor Keith Sisson Masonery, LLC protected their employees from slip / trip hazards while working adjacent to and under the scaffold from debris.

29 CFR 1926.451(h)(1):  Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toe boards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contained or deflected the falling objects:   a) Scaffold in location of Qdoba Mexican Grill, 1001 East Broadway Street, Missoula, MT: On or about October 12, 2018 and at times prior, Sirius Construction, Inc. as the controlling employer, did not ensure that is sub contractor Keith Sisson Masonery, LLC protected their employees from struck by hazards while working adjacent to and under the scaffold structure.

29 CFR 1926.405(b)(2): All pull boxes, junction boxes, or fittings were not provided with covers and in energized installations each outlet box did not have a cover, faceplate, or fixture canopy:   (a) Entry, Qdoba Mexican Restaurant, 1001 East Broadway Street, Missoula, MT: On or about October 12, 2018 and at times prior, Sirius Construction, Inc. did not ensure that employees were protected from shock hazard, in that the outlet face plate cover had been removed to accept the larger gage extension cord to power electrical equipment.
