29 CFR 1910.22(d)(1): Walking-working surfaces were not inspected, regularly and as necessary, and maintained in safe conditions. a) Jobsite: Distribution Warehouse floor - The employer did not inspect and maintain on a regular basis, the surfaces used for powered industrial truck traffic, which consists of damaged holes and bumps. An employee driving a stock picker in a narrow aisle via the wire guided system suffered a severe injury to his lower leg after driving over a hole and losing control of the order picker. Noted on or about 9/25/18.
