29 CFR 1910.303(b)(1):  Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:      (a)  QED at 701 Osage Street, Unit 100, Denver, CO 80204:  On and before October 2, 2018, the employer failed to provide a workplace free from recognized electrical hazards in that electrical cord connected to the foot control on the Take Up machine in the prep area had exposed inner insulation in several locations due to multiple cuts in the outer sheathing. This condition exposed the employees to the hazard of electrical shock.      (b) QED at 701 Osage Street, Unit 100, Denver, CO 80204:  On and before October 2, 2018, the employer failed to provide a workplace free from recognized electrical hazards in that the electrical cord connected to the control box on the Take Up machine in the prep area had been repaired and its outer insulating sheathing was replaced with electrical tape. This condition exposed the employees to the hazard of electrical shock.      (c)  QED at 701 Osage Street, Unit 100, Denver, CO 80204:  On and before October 2, 2018, the employer failed to provide a workplace free from recognized electrical hazards in that knockout plugs were missing on the control boxes to the following equipment:          1.             Stationary Coiler #4, labeled as 240 Volts.  This condition exposed employees to the hazard of electrical shock and burn.      2.             Stationary Coiler #3, labeled as 208-230 Volts.  This condition exposed employees to the hazard of electrical shock and burn.
