29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Butte GM Auto Center, West Wall of Automotive Service Bay: On or about October 11, 2018 and at times prior, employees were exposed to shock and fire related hazards while using a yellow relocatable power tap permanently affixed to the wall as permanent wiring to energize items such as, but not limited to: a water pump. b) Butte GM Auto Center, West Wall of Automotive Service Bay: On or about October 11, 2018 and at times prior, employees were exposed to shock and fire related hazards while using a yellow relocatable power tap wired into an electrical receptacle box to energize items such as, but not limited to: a water pump.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: a) Butte GM Auto Center, Southeast Corner of Automotive Repair Shop: On or about October 11, 2018 and at times prior, employees were exposed to electrical shock and fire hazards while using an orange extension cord as a permanent means of wiring to power items such as, but not limited to: security system equipment.
