Body belts, harnesses, and other components used in personal fall arrest systems, work positioning systems, and travel restraint systems did not meet the requirements of 29 CFR 1910.140.    On or about 10/15/2018, in the warehouse production area at 1030 Raco Court, Lawrenceville, GA:  Employees exposed to a fall hazard at or above four (4) feet from ground level, while standing on the platform of a standup high order picker powered industrial truck was provided a body belt as a personal fall arrest system.

29 CFR 1910.30(a)(1): Before any employee is exposed to a fall hazard, the employer did not provide training for each employee who used personal fall protection systems or who is required to be trained as specified elsewhere in this subpart. Employers did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017. 29 CFR 1910.178(o)(1): Load(s) on powered industrial truck(s) were not stable or safely arranged:    On or about 10/15/2018, in the warehouse production area at 1030 Raco Court, Lawrenceville, GA: Employees using the order picker powered industrial truck for lifting material stacked on the non traditional pallets were exposed to a struck-by hazard.  The employer failed to ensure the pallets used would allow the forks on the lift slide fully through and against the pallet to prevent the material from being unstable and safe when elevated.

OSHA became aware of this employees hospitalized injuries on 10/5/2018.
