29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): Northeast Pallet & Skids, 250 West St., Ludlow, Massachusetts; On and before October 16, 2018, a Mitsubishi powered industrial truck (PIT) was used with damaged and missing clamps to secure the liquefied propane gas (LPG) tank to the PIT and employees were exposed to struck by hazards.

29 CFR 1910.178(m)(5)(i): When a powered industrial truck (PIT) is left unattended, load engaging means were not fully lowered, controls were not neutralized, power was not shut off, and brakes set. New England Pallet & Skids, Inc., 250 West St., Ludlow, Massachusetts; On and about October 16, 2018 an unattended PIT was found with the load engaging means (forks) not fully lowered.
