29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  Villages Of Castleberry Hill Apartments, Building 1100 (Roof): On or about 10/16/2018, and at times prior, the employer exposed employees to struck-by hazards while operating Hitachi Roofing Nailers, in that protective eyewear was not provided to employees installing new shingles with pneumatic tools operating at 120 PSI. Employees were exposed to wire collated fragments, roofing debris, and fasteners.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Villages of Castleberry Hill Apartments, Building #800, Roof: On or about 10/16/2018, and at times prior, the employer exposed employees to falls hazards of approximately 27-feet from the eave to the lower ground level, in that a fall protection system was not provided to employees tearing off roofing shingles from a 6/12 pitched roof.   (b) Villages of Castleberry Hill Apartments, Building #1100, Roof: On or about 10/16/2018, and at times prior, the employer exposed employees to falls hazards of approximately 17-feet, 11-inches to 27-feet from the eave to the lower ground level, in that a fall protection system was not provided to employees installing roofing shingles from a 6/12 pitched roof.
