29 CFR 1910.269(l)(8)(v)(D): The employer did not ensure that each employee exposed to hazards from electric arcs wore protective equipment consisting of at a minimum a face shield with an arc rating greater than or equal to the heat energy estimated under paragraph (l)(8)(ii) of this section whenever that estimate exceeded 2.0 Cal/cm2 but less than 9.0 Cal/cm2.     a) On or about October 15, 2018, at 179 Moore Hill Road, New Windsor, NY, a lineman employee who was installing a new pole, connected an energized transformer line to the wrong energized line resulting in an arc flash explosion. Employee was not wearing face protection and was exposed to a heat energy level of 4.0 Cal/cm2.
