29 CFR 1910.27(b)(2)(vi): 29 CFR 1910.27(b)(2)(vi) The employer must ensure that each employee uses a separate, independent personal fall arrest system that meets the requirements of subpart I of this part: On or about October 17, 2018 an employee using a rope descent system to clean windows from was exposed to fall hazards and the employer did not ensure that his personal fall arrest system met the requirement of subpart I, in that the life line was not anchored appropriately.

29 CFR 1910.140(c)(12): Anchorages used to attach to personal fall protection equipment were not independent of any anchorage used to suspend employees or platforms on which employees work. Anchorages used to attach to personal fall protection equipment on mobile work platforms on powered industrial trucks were not attached to an overhead member of the platform, at a point located above and near the center of the platform. On or about October 17, 2018 employees engaged in window washing activities were exposed to fall hazards and the employer did not ensure that anchorages for their personal fall arrest systems were independent from the anchorage used by the rope descent system.
