29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) 3212 Emerson Way Mechanicsburg, PA - Employees were standing on a steep residential roof, with an 8 in 12 pitch installing PVC pipe for an HVAC unit on the side of the residential home without the benefit of a complete fall protection system, thereby exposing employees to a fall of 11 feet to the ground below, on or about October 17, 2018. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) 3212 Emerson Way Mechanicsburg, PA - The employees were using an aluminum extension ladder to access the residential roof. The upper portion of the ladder did not extend three feet above the landing, on or about October 17, 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
