29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard were being followed. On or about 10/16/2018, Standley Batch Systems, Inc. did not conduct periodic inspections on their authorized employees or their procedures for several machines to include, but not limited to a horizontal band saw, milling machine, ironworker, drill press and overhead crane.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. On or about 10/16/2018, employees from Standley Batch Systems, Inc. were exposed to hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. The hold-down clamp on the plate shear portion of the machine was stripped near the point of operation and the punch guard was improperly installed on a Central Engineering Corporation Metal Muncher Ironworker, Model # MM 61BC, Serial # 1313-9-76. Standley Batch Systems, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(iii) which was contained in OSHA inspection number 949228, citation number 1, item number 7, issued on 11/27/2013, and was affirmed as a final order on 12/17/2013, with respect to a workplace located at 505 Aquamsi Street, Cape Girardeau, MO 63703.
