29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about October 11, 2018, and in times prior thereto, while in manual mode, a procedure for locking out the reactor when by-passing the inter-locked safety door and opening the job-made lower doors for troubleshooting was not developed and utilized, exposing employees to the hazards of being caught in moving parts. This hazard was located in the reactor area.  b) On or about October 11, 2018, and in times prior thereto, a steel brush was provided and used to by employees to clean plastic residue from the jaws of a reactor without developing a procedure to lock out the reactor, exposing employees to the hazards of being caught in moving parts. This hazard was located in the reactor area.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about October 11, 2018, and in times prior thereto, the plastic guard covering the lower half of reactor 3290 had been modified by a sub-contractor under the direction of the employer. The plastic barrier provided by the manufacturer was cut in half right through the warning decal to make two doors. This allowed for workers to reach into the machine during operation to remove plastic bags. The new doors were not interlocked as the ones provided by the manufacturer, thus exposing operators to the hazard of amputation. This hazard was located in the Freedom reactor area.

29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) for easy handling of material without the operator placing a hand in the danger zone were used in lieu of other guarding required by this section, but not only used to supplement protection required:  On or about October 11, 2018, and in times prior thereto, a steel brush was provided and used to clean plastic residue from the jaws of a reactor while it was in operation exposing operators to an amputation hazard, located in the Freedom reactor area.
