29 CFR 1910.303(b)(1)(viii): Electrical equipment was not free from recognized hazards based on other factors that contribute to the practical safeguarding of persons using or likely to come in contact with the equipment: GB Fabrication Company - Shiloh, Ohio: On or about October 12, 2018, electrical equipment associated with the hoists was not kept free from hazard in that water leaked from the roof onto the hoist equipment. Employees who operated the hoists to load the laser tables were exposed to electrical hazards.

29 CFR 1910.332(b)(2): Employees, who were not qualified persons, were not trained in and familiar with any electrically related safety practices not specifically addressed by 29 CFR 1910.331 through 29 CFR 1910.335, but which were necessary for their safety. GB Fabrication Company - Shiloh, Ohio: On or about September 1, 2018, the employer did not ensure employees who performed electrical work were trained in and familiar with safety related work practices. Employees that performed tasks such as, but not limited to, accessing electrical panels for cleaning operations were exposed to electrical hazards in that the equipment was not locked out.

29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were deenergized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2): GB Fabrication Company - Shiloh, Ohio: On or about September 1, 2018, the employer did not ensure that employees who accessed the electrical distribution panel of the LVD 0327 (s/n 22616) press brake were protected from electrical hazards. A lockout device was not applied to the disconnecting means by each employee who accessed the equipment's 480 volt electrical distribution panel prior to performing cleaning tasks.
