29 CFR 1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position:  On or about October 11, 2018, jobsite located at 1750 S Bering St., Dania Beach, FL 33004, the employer exposed an employee to a fall hazard of approximately 24-feet above the lower level while employee was hanging a piece of plywood onto a parapet wall while he had climbed on the mid rails on the edge of the basket and not standing firmly on the floor of the basket.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  On or about October 11, 2018, jobsite located at 1750 S Bering St., Dania Beach, FL 33004, the employer exposed employees to fall hazards of 24-feet above the lower level, while operating an aerial lift without a harnesses and/or without being tied to the boom or basket.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems:  On or about October 11, 2019, jobsite located at 1750 S Bering St, Dania Beach, FL 33004, employees engaged in roofing work were exposed to approximately a 24-foot fall hazard on an existing commercial structure with a flat roof without means of fall protection.
