29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: a) Alumi-Pole Pump Jack Scaffolding System: On or about October 16, 2018 a guardrail system was not installed on both ends of the scaffold platform that was positioned more than 10 feet above ground level. b) Alumi-Pole Pump Jack Scaffolding System: On or about October 16, 2018 a mid-rail was not installed halfway between the platform and the top-rail on the open side away from the building structure.
