On 10/16/2018, Domingo Soloman was replacing roofing materials on a residential apartment building located at 38-49 Oak Knoll Drive in Festus, MO. The ladder the employees were using to access the second story roof was not extended three feet beyond the landing or edge of the roof.

Domingo Soloman was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1326473, citation number 1, item number 2, issued on 7/12/2018, and was affirmed as a final order on 8/08/2018, with respect to a workplace located at 317 Trotter Drive in Hamel, IL.
