29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): Two employee were not protected from cave-in hazards while working in an approximately 30 foot long by 3 foot wide and 7 foot deep excavation. Tiger Plumbing Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.651(a)(1), which was contained in OSHA inspection number 1353968, citation number 01, item number 03 and was affirmed as a final order on October 17, 2014, with respect to a workplace located at 217 S St. Louis Street in East Alton Illinois.
