29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) 1532 East Matisse Drive Lot #6 - Observed on or about 17 OCT 2018, the employer failed to ensure that employees conducting roofing operations were tied off at all times, and adequately protected from fall hazards while working approximately twenty-five (25) feet above grade.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a) 1532 East Matisse Drive Lot #6, Middletown, Delaware - On or about 17 OCT 2018, the employer failed to ensure that each employee on the roof was trained to recognize, evaluate, and mitigate hazards associated with working on roofs.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:    a)  1532 East Matisse Drive Lot #6 - The employer failed to ensure that personal fall arrest systems were securely fastened to the anchor point by the attachment point located in the center of the wearer's back.  Employees were connected via a leg strap below the waist.
