On or about October 11, 2018, the employer did not ensure energy control procedures were utilized when a maintenance worker performed service and maintenance (inspecting) operations of the Evisceration Line #5 Drive Motor.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about October 11, 2018, the employer did not ensure the established procedure for the application of energy controls (lockout/tagout) were done in sequence while a maintenance man was inspecting the Evisceration Line #5 Drive Motor. An employee did not apply energy controls by; turning off and/or shutting down the evisceration line [per the 1910.147(d)(2) requirements], isolating all energy sources [(d)(3)], affixing a lockout device to the energy isolating device [(d)(4)], and verify the equipment energy was isolated and de-energized prior to starting work [(d)(6)].

29 CFR 1910.147(c)(6)(i)(C): Where lockout is used for energy control, the periodic inspection shall include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected. On or about October 11, 2018, the employer did not ensure periodic inspections of energy control procedures included a review of each authorized employees responsibilities under the energy control procedures. Authorized employees are required to conduct service and maintenance activities on the Chicken Evisceration Line requiring them to perform lockout/tagout.
