29 CFR 1910.95(g)(5)(i): The employer did not establish, within 6 months of an employee's first exposure at or above the action level, a valid baseline audiogram against which subsequent audiograms can be compared:    Instances:  a. On or about October 16, 2018, at 2640 N Powerline Road, Pompano Beach, FL 33069 grinding department, an employee operating a grinder and belt sander was exposed to a noise level of 87.5dBA, which exceeds the 85.0 dBA during the 8-hr representative sampling period. The employer failed to provide the employee with a baseline audiogram within six months of an employee's first exposure above the action level.     b. On or about October 16, 2018, at 2640 N Powerline Road, Pompano Beach, FL 33069 grinding department, an employee operating a grinder was exposed to a noise level of 91.1dBA, which exceeds the 85.0 dBA during the 8-hr representative sampling period. The employer failed to provide the employee with a baseline audiogram within six months of an employee's first exposure above the action level.    c. On or about October 16, 2018, at 2640 N Powerline Road, Pompano Beach, FL 33069 grinding department, an employee operating a tumbler was exposed to a noise level of 88.9dBA, which exceeds the 85.0 dBA during the 8-hr representative sampling period. The employer failed to provide the employee with a baseline audiogram within six months of an employee's first exposure above the action level.    d. On or about October 16, 2018, at 2640 N Powerline Road, Pompano Beach, FL 33069 grinding department, an employee operating a grinder was exposed to a noise level of 88.8dBA, which exceeds the 85.0 dBA during the 8-hr representative sampling period. The employer failed to provide the employee with a baseline audiogram within six months of an employee's first exposure above the action level.

29 CFR 1910.133(a)(3): The employer did not ensure that employees wearing prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design or wears eye protection that can be worn over the prescription lenses:    On or about October 16,2018, at 2640 N. Powerline Road, Pompano Beach, FL 33069 grinding and finishing department, the employer did not provide adequate eye protection for those employees who wore prescription glasses.
