On or about October 16, 2018, at 2640 N Powerline Road, Pompano Beach, FL 33069 grinding department, an employee operating a grinder and belt sander was exposed to a noise level of 87.5dBA, which exceeds the 85.0 dBA during the 8-hr representative sampling period. The employer failed to provide the employee with a baseline audiogram within six months of an employee's first exposure above the action level.     b. On or about October 16, 2018, at 2640 N Powerline Road, Pompano Beach, FL 33069 grinding department, an employee operating a grinder was exposed to a noise level of 91.1dBA, which exceeds the 85.0 dBA during the 8-hr representative sampling period. The employer failed to provide the employee with a baseline audiogram within six months of an employee's first exposure above the action level.    c. On or about October 16, 2018, at 2640 N Powerline Road, Pompano Beach, FL 33069 grinding department, an employee operating a tumbler was exposed to a noise level of 88.9dBA, which exceeds the 85.0 dBA during the 8-hr representative sampling period. The employer failed to provide the employee with a baseline audiogram within six months of an employee's first exposure above the action level.    d. On or about October 16, 2018, at 2640 N Powerline Road, Pompano Beach, FL 33069 grinding department, an employee operating a grinder was exposed to a noise level of 88.8dBA, which exceeds the 85.0 dBA during the 8-hr representative sampling period. The employer failed to provide the employee with a baseline audiogram within six months of an employee's first exposure above the action level.
