29 CFR 1926.503(c): When the employer had reason to believe that any affected employee who had already been trained did not have the understanding and skill required by paragraph (a) of this section, the employer did not retrain each such employee: a) Job site, 1290 Western Avenue, Pittsburgh, PA: On or about October 17, 2018, during construction activities, the employer did not ensure that appropriate fall protection systems/devices were in use in that a safety monitor system was not properly and effectively in use as intended, flag lines were removed form the rear the roof and existing parapet walls were not of adequate height to serve as fall protection. b) Job site, 1290 Western Avenue, Pittsburgh, PA: On or about October 17, 2018, during construction activities, the employer did not ensure that appropriate fall protection systems/devices were in use in that flag lines used a dull red colored yarn/similar type material on them that was not highly visible, as it blended in with the surrounding background.
